Case Management

The case management department at Mammoth Hospital provides many diverse services for our patients, community members and employees.  It is our mission to treat the whole patient and help them re-establish themselves in the home and community after discharge from the hospital. The following are just a few of the many services provided by Mammoth Hospital’s Case Management Department.  If you need social services assistance, please call Natalie Sanders,RN at (760) 924-4151.

Discharge Planning:  We can set up Home Health, Medical equipment and oxygen, and follow- up visits with specialists. We can also help with placement in skilled nursing facilities, orthopedic rehab or drug and alcohol rehab.

Social Issues:  We facilitate referrals to appropriate agencies for abuse cases, whether it be sexual, domestic violence, child or elder abuse.

Grief Counseling:  We have a grief support group that meets the 1st and 3rd Thursday of every month in our new "Support Room" in the new building at 12:30pm.

Cancer Outreach:   Our cancer support group meets the 1st and 3rd Wednesday of every month in our new "Support Room" in the new building at 12:30pm.

Hospice:  We have trained volunteers who go out into homes of the terminally ill patients to provide moral support for the family and patients. We also provide respite care so family members or care givers can get away for brief periods.

Indigent Services:  Case Management provides indigent patients help with food, clothing, lodging and travel.

Family Support:  Case Management provides help with care of family members whether adult, children or pets that are in need of help due to their loved ones injury or illness. 

Travel:  We can help with travel arrangements for patients.
